Job Description

NHS Lanarkshire is seeking a Community Link Worker to join a Primary Care multi-disciplinary team. You will provide a patient-centred approach, helping patients identify issues to improve their lives and navigate pathways to engage in meaningful activity.

You will support access to community services, promote health improvement, and work with care teams to enable decision-making. Some home visits and flexible hours may be required.
