Role Overview

Inverclyde HSCP ANP service delivers urgent and unscheduled care on behalf of GP Practices through a dedicated home visiting model. The role supports care home residents, housebound patients and those too acutely unwell to visit their GP. Working closely with GPs and multidisciplinary teams, ANPs provide seamless, high quality, safe care within the community, collaborating with carers and social care colleagues. They manage a wide range of conditions, carry out comprehensive clinical assessments, arrange investigations, develop treatment plans, prescribe where appropriate, and coordinate transfer to hospital when required. The service also promotes health education, rehabilitation, and wellbeing, helping patients maintain independence at home.

Duration, Location and Working Pattern

  • This is a permanent post working 27 hours per week (various hours are available).
  • Shift pattern will be discussed; shift typically commences at 8:30 a.m.
  • Based at Port Glasgow Health Centre, Bay Street, Port Glasgow.
  • Work across roles and geographical boundaries, collaborating with health and social care professionals throughout Inverclyde.
  • Salary is pro rata for part-time hours.

Key Responsibilities

  • Deliver comprehensive clinical care within a multidisciplinary team and Single Point of Access model, responding to GP home visit requests, assessing, diagnosing, and managing patient needs autonomously and collaboratively.
  • Provide evidence based care, clinical leadership and guidance to nursing and multidisciplinary colleagues, supporting unregistered staff.
  • Contribute to setting, implementing, and evaluating standards of nursing practice, ensuring high quality care for residents across Inverclyde.
  • Collaborate with GPs, NHS 24, and the Scottish Ambulance Service for safe, coordinated patient care.

Qualifications, Training and Experience

  • Master’s level qualification in Advanced Practice.
  • Independent/Supplementary Prescriber registration (V300) with the Nursing and Midwifery Council.
  • Relevant clinical experience, including completion of Clinical Assessment, Clinical Reasoning, Judgement and Diagnostic Decision Making, Anatomy and Pathophysiology, Non medical Prescribing (V300), Leading, Delivering and Evaluating Care and Practice Learning/Transferable Work Based Learning – community experience desirable.
  • Interest in palliative care, long term conditions and anticipatory care is advantageous.
  • Full driving licence essential.
  • Experience in pre employment checks, PVG Scheme Membership and Disclosure Scotland Check required.
