We are seeking a pharmacist who is ambitious and keen to combine an expert clinical role with a chance to develop commercial and leadership skills.

As a Screen Clinical Pharmacist you will:

Work collaboratively with GP Practices in a hybrid role, applying your expertise as a medicines specialist to enhance patient care through clinical improvement projects and patient review services.

Proactively identify patients requiring pharmacological and non-pharmacological interventions, supporting the development and implementation of evidence-based treatment and management plans in partnership with Practice teams.

Expand and apply your clinical knowledge across a broad range of therapy areas, contributing to prescribing optimisation programmes that improve safety, effectiveness, and overall patient outcomes

Qualifications and essential criteria:

Qualified pharmacist

GpHC registration

Full UK Driving License and access to a car

A proactive, reliable and detailed focused approach

Confident in Microsoft Office

Willing to travel as required along with remote home working.
