Support Practitioner

Service: Angus

Based: Montrose

Hours: Full Time, Part Time & Relief Available

Contract: Permanent

Salary: £25,878 (pro rata)

We are looking for individuals to provide person centred support to adults with complex needs, including learning disabilities and autism, in their homes and within the community.

Role responsibilities include:

• Deliver 1:1 support to adults with complex needs, promoting independence and choice.
• Facilitate daily living tasks, including cooking, housekeeping and safe living practices.
• Encourage participation in education, employment, volunteering and social activities.
• Assist clients in developing and maintaining relationships.
• Work collaboratively with health, social work, community justice, police and advocacy teams.
• Document care plans and progress according to organisation standards.

Qualifications & Requirements:

• No previous work experience required; full training provided.
• IT skills required for all vacancies.
• Ability to register with the SSSC within 6 months of start date where applicable.
• Valid UK driving licence and access to a vehicle (if required).
• Knowledge of health and social care services is an asset.
