1.0 wte (36hrs)

Responsibilities

Provide physiotherapy services to adults, mainly older people, within the Community Rehabilitation Team, working in patients’ own homes, a homely setting or as an outpatient. Focus on community rehabilitation, preventing unnecessary hospital admission, supporting early discharge from hospital and enabling patients to remain as independent as possible in their homes. The role may involve working at other community hospital sites.

Qualifications

• Degree in Physiotherapy and registration with the HCPC. • NHS experience working with adults, primarily older people. • Assessment and treatment knowledge and skills associated with this patient group. • Evidence of continuous professional development appropriate for Band 6. • Evidence of involvement in audit and service development. • Experience of supervising junior staff at Band 6 level.

Additional Information

Public holiday working in operation (including Christmas Day and New Year’s Day). Ability to travel throughout the area is essential as part of the community based post.
