Adult Complex Care Team – Band 3 HCSW
NHS Grampian is seeking compassionate and motivated Healthcare Support Workers to join our Adult Complex Care Team, supporting young adults with complex, life limiting conditions in their own homes following transition from paediatric care.
About the Role
As part of our dedicated team you will deliver high quality, evidence based person centred care to young adults with complex physical health needs and learning disabilities. You will work closely with families, guardians and a wide range of professionals to create a supportive, inclusive environment that enables individuals to live safely and comfortably at home.
This role requires working autonomously, making safe clinical judgments and escalating any concerns appropriately. Strong communication skills are essential due to the independent nature of the post. Experience with tracheostomy care and ventilation is highly advantageous.

- Relevant experience working as a Healthcare Support Worker
- Ability to work independently and as part of a wider multidisciplinary team
- Strong interpersonal and communication skills
- A valid UK driving licence and the ability to drive patient vehicles
