Gracemount Medical Group is looking for an enthusiastic and motivated Practice Nurse to join our growing multidisciplinary team in South East Edinburgh.
We are a progressive and developing practice caring for over 10,000 patients. Our priority is to provide high-quality, personalised and accessible care while maintaining continuity and continually improving the way we work.
Our clinical team includes GP Partners, salaried GPs, Advanced Nurse Practitioners, General Practice Nurse and a Healthcare Assistant, alongside a growing multidisciplinary team including Clinical Pharmacists, Physiotherapy and Mental Health services.
We have worked hard to develop a supportive team culture and encourage all members of the practice to contribute ideas about how we can improve care for our patients and make our systems work better for both patients and staff.
We are looking for a Practice Nurse who enjoys working autonomously while also being part of a supportive multidisciplinary team.
The role will have a particular focus on long-term condition management, preventative healthcare and women’s health.
Depending on the successful candidate’s experience and interests, responsibilities may include:
- Long-term condition reviews, including respiratory and cardiovascular disease utilising Medlink software
- Cervical screening
- Health promotion, prevention and lifestyle support
- Women’s health
- Immunisations and injections where appropriate
- Participating in clinical meetings, audit and quality improvement
- Working collaboratively with the wider practice and multidisciplinary team
Experience or additional qualifications in areas such as:
- Asthma and COPD management / CVD / Diabetes / Women’s Health / Health promotion
- General Practice or Primary Care nursing
- Non-medical prescribing
