Working Hours:16 hours weekly

Working days: Preferable Wednesday / Friday

About Us:

  • Serving approximately 8,500 patients
  • Team includes:
    • 5 GP Partners
    • 3 Salaried GPs
    • 1 Advanced Nurse Practitioner
    • 2 Practice Nurse
    • 1 Chronic Disease Nurse
    • 2 Healthcare Assistants
    • Pharmacy team
    • First Contact Physiotherapist
  • Supported by an experienced administrative team
  • Day to day operations overseen by the Practice Manager and Assistant Practice Manager

The Role: This is an exciting opportunity for a Pharmacist to work as part of a multidisciplinary primary care team, supporting the safe, effective, and cost efficient use of medicines. The successful candidate will provide clinical input into patient care, support prescribing processes, and contribute to improving outcomes for our patient population.

You will work closely with GPs, nurses, and the wider team to undertake medication reviews, manage repeat prescribing, and support patients with long term conditions.

What we’re looking for:

Pharmacist with an independent Prescribing Qualification

  • Strong clinical assessment and diagnostic skills
  • Excellent communication and interpersonal abilities
  • Ability to work autonomously and as part of a team

Key Responsibilities:

  • Undertake structured medication reviews
  • Process and manage repeat prescription requests safely and efficiently
  • Provide clinical support and prescribing advice to GPs and the wider practice team
  • Support patients with long term conditions, ensuring appropriate medication use
  • Review and action hospital discharge summaries and clinical correspondence
  • Identify and address medication related queries and safety issues
  • Contribute to clinical audits, quality improvement, and prescribing initiatives
  • Promote cost effective prescribing in line with local and national guidance
  • Provide patient education and advice on medication use and adherence
  • Support the practice in meeting NHS and prescribing targets
