Role Overview
We are looking for an enthusiastic and innovative Physiotherapist to join the Falkirk Health and Social Care Partnerships ReACH Service. The ReACH Teams provide multidisciplinary assessment and rehabilitation for adults in the community.
Key Responsibilities
- Perform physiotherapy assessment, diagnosis and treatment of patients within the community, in line with the National Discharge without Delay programme.
- Prevent unnecessary hospital admission and support earlier hospital discharge.
- Make senior clinical decisions, lead complex care and provide referrals to other services as required.
- Support the development of less experienced and unregistered staff, contributing to practice education and service improvement.
- Act as a Forth Valley wide resource for advice on treatment and management of complex patients in partnership with other highly specialist AHPs.
- Deliver services on a 7 day schedule, including weekend work.
Qualifications and Experience
- Broad experience in community rehabilitation, frailty and falls prevention.
- Highly developed skills in community rehabilitation or frailty and falls prevention.
- Proven record of service development.
- Band 7 band experience is desirable.
- Must be a member of the Protecting Vulnerable Groups (PVG) Scheme.
Working Conditions
Base at Falkirk Community Hospital with the ReACH Falkirk Team. The post includes 7 day service with weekend working. From 1 April 2026 the Agenda for Change full time working week will be reduced from 37 to 36 hours per week; part time hours will be reduced pro rata but the hourly rate will increase so pay will not be affected.
