The Mill Practice is seeking an enthusiastic and forward-thinking General Practice Pharmacist to join our multidisciplinary team. This is an exciting opportunity to develop a pharmacist-led service within an established and progressive GP practice committed to delivering high quality, patient centred care.

Working alongside GPs, Practice Nurses and our wider mult-disciplinary clinical and administration teams, you will play a key role in improving prescribing safety, reducing medication related harm and helping patients achieve the best possible outcomes from their medicines.

The Role

The successful candidate will:

  • Undertake structured medication reviews for patients with polypharmacy and complex health needs.
  • Lead medicines optimisation initiatives across the Practice.
  • Support safe and effective repeat prescribing processes.
  • Manage Chronic Medication Service (CMS) and serial prescribing workflows.
  • Provide prescribing advice and support to clinicians and practice staff.
  • Deliver pharmacist-led clinics for long term conditions where appropriately trained.
  • Support prescribing quality improvement, audit and patient safety initiatives.
  • Work collaboratively with care homes and community healthcare professionals.
  • Contribute to the development and expansion of pharmacy services within the Practice.

About you

We are looking for an individual who can demonstrate:

Essential

  • Current registration with the General Pharmaceutical Council (GPhC).
  • Experience undertaking clinical medication reviews.
  • Excellent communication and interpersonal skills.
  • Strong clinical assessment and decision-making abilities.
  • Commitment to patient-centred care.
  • Ability to work independently and as part of a multidisciplinary team.

Desirable

  • Independent Prescriber qualification.
  • Experience working in General Practice or Primary Care.
  • Experience managing patients with long-term conditions.
  • Knowledge of medicines optimisation and prescribing governance.
  • Experience of audit, quality improvement and service redesign.
