Job Description

Job PurposeThe Community Navigator supports disabled people, unpaid carers and people with long-term conditions to access services, opportunities, and community resources that improve independence, wellbeing, and social inclusion. The role focuses on person centred support, empowerment, and reducing barriers that prevent people from living full and connected lives.

Key Responsibilities

  • 1. Person-Centred SupportWork one-to-one with people to identify goals, needs, and prioritiesPractical, hands-on assistance to support people with paperwork, attend appointments or a support group, online applications and build confidence to support independence.Provide practical guidance to access services, benefits, community activities, and supports
  • 2. Navigation & AdvocacyHelp people understand and navigate health, social care, housing, welfare, and community supports.Support people to self-advocate and build confidence Signpost and refer to appropriate services and opportunities
  • 3. Community ConnectionBuild knowledge of local resources, groups, and supportsDevelop relationships with community organisations and partnersPromote inclusive opportunities that reduce isolation and increase participation
  • 4. Partnership WorkingLiaise with statutory, third sector, and community providersAttend multi-agency meetings where appropriateContribute to collaborative approaches that improve outcomes for individuals
  • 5. Monitoring & ReportingMaintain accurate, confidential case recordsRecord outcomes and impact using agreed systemsContribute to evaluation, reporting, and service improvement
  • 6. Safeguarding & Professional PracticeFollow safeguarding, confidentiality, and data protection proceduresWork within organisational policies and professional boundariesParticipate in supervision, training, and reflective practice
