Community Mental Health Nurses

We are looking for two enthusiastic Community Mental Health Nurses with Older Adult Mental Health experience to join the Caithness Community Mental Health team based at Town & County Hospital, Wick. The role will involve working with adults over the age of 65 or patients of any age with a memory concern.
Responsibilities
- Conduct assessments, develop and implement care plans, and evaluate outcomes in partnership with clients, families, and carers.
- Manage ongoing risk and coordinate medication monitoring.
- Collaborate with psychiatrists, primary care and other social care services, and refer patients to other services as appropriate.
- Act as an active member of the multi-disciplinary team and supervise students and support workers on occasion.
- Travel efficiently to undertake the role, using a personal vehicle if required.
Qualifications
- Registered with the National Nursing and Midwifery Council (NMC) and compliant with the code of conduct.
- Experience in older adult or community mental health nursing.
- Full driving licence and willingness to travel by car to meet role requirements.
There are two permanent posts available totalling 51.4 hours, hours negotiable.
