University Hospitals Sussex NHS Foundation Trust seeks a Grade Band 6 Acute Frailty Nurse for a permanent, full time (37.5h/week). Hours are Monday to Friday with the expectation of participating in a 7 day rota when the service expands. Salary: £39,959 £48,117 per annum.

Main duties of the job
The post holder will:
- Manage triage of patient referrals, ensure timely interventions, and handle admissions and discharges.
- Delegate workload and prioritise patient care activities.
- Conduct audits relevant to nursing professional development and monitor standards of care.
- Act as an agent for change, promoting a culture of flexibility and improvement.
- Ensure confidentiality and maintain accurate documentation and records.
- Work within an unpredictable environment, addressing aggression, violence and clinical emergencies.
- Participate in the unit’s privacy and dignity agenda.
Successful candidates will be expected to work a 7 day rota when the service becomes 7 day.
Detailed job description and main responsibilities
- Responsible for delivery of patient care in the specialist frailty area.
- Initiative to undertake duties within the specialty, including responding to adverse reactions.
- Comprehensive assessment of patients, using knowledge and experience.
- Develop individual patient led treatment plans based on clinical reasoning and evidence.
- Coordinate multidisciplinary interventions through case management, MDT meetings and community teams.
- Maintain complete and accurate documentation; update databases and records.
- Implement care sessions based on sound theory and evidence.
- Use clinical governance frameworks; participate in supervision and reflection.
- Apply clinical judgement to evaluate patient care options.

Person specification Skills and Knowledge
- Registered nurse with current UK NMC registration.
- Substantial experience in frailty care.
- Competent in venepuncture and cannulation.
- TEACH module participation or progression.
- Competent in blood transfusion and IV therapy administration.
- Evidence of professional development.
- Evidence of broad clinical skills.
- Leadership courses or proven leadership experience.
