An exciting opportunity has arisen for a number of new Advanced Clinical Practitioner Roles, inclusive of nurses and AHPs, to join our Advanced Practitioner team within Perth & Kinross Health & Social Care Partnership as part of our Locality Integrated Care Team across a range of community settings.
The Advanced AHP Practitioner role offers the opportunity to develop their career in advanced practice; improving patient outcomes and pathways of care for frail, older adults in the community.
The post holders will demonstrate excellent leadership and communication with the ability to work autonomously across the four pillars of advanced practice. In addition to bringing profession specific expertise to the role, applicants will provide advanced clinical practice; through comprehensive patient history taking, advanced clinical assessment, differential diagnosis, problem solving and treatment planning, incorporating non-medical prescribing (if NMC falls in line with their professional scope of practice).
The post holders will play a key role in the clinical leadership and development of advanced practice within the community, delivering high quality evidence based and person-centred care.
They will pro-actively manage frail elderly adults and you will be expected to act as a clinical leader working across general practice and community hospitals in close collaboration with the Integrated Care Teams, General Practitioners and Medicine for the Elderly Consultants.
This role will be managed and clinically supported by the Lead Advanced Nurse Practitioners. Post holders will also be supported to maintain appropriate professional supports and work collaboratively with all members of the community multidisciplinary teams. Travel across Perth and Kinross is necessary to undertake the role.
The applicant must evidence extensive, relevant and varied post registration CPD and experience within an appropriate the clinical field. They must also have a proven ability to work at Masters Level learning in Advanced Practice. Candidates who do not meet the Advanced Practitioner criteria are welcome to apply for the trainee role and will be fully supported to undertake the required education and supervised practice to develop the competence and confidence required to work as an Advanced Practitioner. It is an expectation that as a trainee you will successfully achieve the required qualifications as an Advanced Practitioner within the first 18 to 24 months of appointment.

Hours of work are: 37 Part Time to a minimum of 0.8 may be considered.
