Job Description

Wound & Dressings Clinic & Vaccination Nurse To provide and maintain a high standard of nursing care for patients. Provide nursing assistance to the GPs and other members of primary healthcare team, whilst providing supervision and working closely with dressings clinic Healthcare Assistant. The duties will, in time and with training where necessary, include wound assessment and dressing application in clinic, and vaccination of housebound patients. The role is based across a local cluster of 3 General Practices as part of the PCN:

Eden Medical Group

Carlisle Central Practice

Warwick Square Group Practice

The nurse will have skills, training and experience, relating to wound management and dressing application. They will be responsible for the administration of vaccinations to housebound patients. They will undertake independent consultations with patients relating to specific symptoms within the post holders scope of practice; assessing, examining, investigating, diagnosing (with support where needed), treating, and following up patients as required.

The nurse will use clinical decision making within their scope of practice and be responsible for leading the dressings clinic and housebound vaccinations for the PCN Practices. They will ensure the delivery of evidence-based care in relation to tissue viability including leg ulcer management.

Main duties of the job Duties include, but not limited to

Perform holistic assessment and provide advice, care planning and treatment for patients with leg ulcers following referral from health care professionals.

Plan, implement and evaluate care with patients and carer using diagnostic tests where appropriate, e.g. Doppler.

Provide advice on the most appropriate wound dressing products.

Provide ongoing assessment and management of venous disease.

Assess for and advise re compression bandaging and hosiery.

Standardise, and encourage optimal practice in, care with the goal of improving patient outcomes.

Encourage individualised care that enables people to tolerate treatments and thereby improve outcomes.

Agree treatment goals with the patient and a time frame for their achievement.

Provide advice on management of slow healing wounds.

Undertake routine vaccinations of our housebound population, driving licence essential.

This role is intended to become an integral part of the PCNs multidisciplinary team, encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN. There will be a requirement to work with a wide range of professionals and partner organisations.

Governance

Undertake DBS check at regular intervals

Able to recognise and manage anaphylaxis accordingto current UK guidelines

Able to perform cardio-pulmonary resuscitationaccording to current UK guidelines

Ability to obtain and document informed consent(either verbal or written)

Ensure infection control guidelines are maintainedand contribute to infection control audits and policies

Ensure awareness of statutory and local protectionprocedures, including systems of referral

Support the Practice clinical governance agenda

Identify changes to clinical practice that arerequired to implement evidence-based guidelines

Produceaccurate, contemporaneous and complete records of patient consultation,consistent with legislation, policies and procedures

Workwithin policies regarding family violence, vulnerable children and adults, andall aspects of Safeguarding, substance abuse and addictive behaviour, and referas appropriate

Aware of identification and reporting proceduresrelated to professional standards

An understanding, acceptance and adherence to theneed for strict confidentiality

To work only in accordance with the UKCC Code ofConduct and within the scope of professional practice

To have a written professional development plan andto maintain an up to date portfolio which meets the requirements ofregistration with the UKCC

Cooperate with annual appraisal meetings
