Nurse Practitioner, Hospital at Home – Acute Care Leader

Job Description

NHS Forth Valley is inviting applications from a highly motivated Nurse Practitioner to join the Hospital at Home Team. The post involves supplementing Medical staff to manage acute admissions and to provide high-quality, streamlined, evidence-based care.

You will assess care needs, initiate timely interventions, and support discharge planning within a multidisciplinary team. Training will be provided, with supervision by Band 7 ANPs and opportunities for ongoing professional development.
