Role Summary

Lead the delivery of intensive case management and pre and post detox support for individuals experiencing alcohol problems across South Lanarkshire. Work independently, using an assertive outreach approach to engage service users where they are and provide structured, person centred interventions.

Responsibilities

  • Assertive outreach to individuals in the community
  • Intensive person centred case management
  • Develop and implement personalised support plans
  • Deliver structured psychosocial interventions (e.g., ABI, motivational interviewing, crisis support, family support)
  • Provide harm reduction advice and aim based alcohol management plans
  • Use alcohol screening tools to identify problem and dependent use
  • Facilitate linkage with community resources, health professionals and CAReS services
  • Support engagement with peer networks and recovery services
  • Participate in multi agency reviews and community events
  • Offer pre detox preparation and post detox community support, including residential rehab pathways
  • Deliver information and advice to help service users make informed choices about alcohol use

Qualifications

  • Experience in the Alcohol and Other Drugs sector
  • Strong commitment to respect, compassion, inclusion and integrity
  • Full UK driver’s licence and access to a vehicle (for travel within South Lanarkshire); paid mileage at 45p per mile
  • IT skills – competent use of NHS and service specific information systems
  • Willingness to register with the Scottish Social Services Council (SSSC) within six months of start date
