An exciting opportunity has arisen to join NHS Greater Glasgow and Clyde as a Crisis Practitioner within the South Glasgow Mental Health Crisis Team, where you will play a vital role in delivering compassionate, responsive care to individuals in acute distress. Working within one of the largest NHS organisations in the UK, you will be part of a supportive, forward thinking multidisciplinary team committed to high quality, person centred care. This dynamic role offers the chance to make a real difference by supporting people in their own homes as an alternative to hospital admission. You will work closely with inpatient and community services to support recovery and facilitate early discharge. Duration, Location, and Working Pattern:
- Full Time, Permanent (36hrs)
- Base Location: Florence Street Mental Health Resource Centre, Glasgow
- Shift Pattern: Operates over 7 days between 0900hrs and 2000hrs Mon Fri, and hrs Sat Sun
- Long days are available.
Key responsibilities:
- Provide ongoing assessment, care planning, intervention and evaluation for individuals experiencing acute mental health crisis
- Deliver intensive home treatment as an alternative to hospital admission, supporting recovery in the community
- Undertake comprehensive risk assessments and implement effective risk and safety management strategies
- Work collaboratively with GPs, CMHTs, inpatient services, specialist services and third sector partners
- Support early discharge from the hospital through effective discharge planning and follow up care
- Supervise, support and mentor junior staff and students, contributing to their development and practice
- Maintain accurate clinical records and ensure care is delivered in line with legal, ethical and professional standards
Knowledge, training, qualifications and/or experience required to do the job:
- Registered Mental Health Nurse with NMC registration or Registered Occupational Therapist with relevant UK registration
- Post registration experience within mental health services, including experience managing complex cases
- Demonstrable experience in risk assessment and management within acute or community mental health settings
- Excellent communication skills with the ability to engage effectively across multidisciplinary teams and stakeholders
- Knowledge of mental health legislation, policy and best practice with commitment to continuous professional development
- Ability to work autonomously in a fast paced environment while contributing effectively within a team
- Full UK driving licence with access to a vehicle and basic IT skills for record keeping and communication
