Role Overview

A Health and Social Care Coordinator (HSCC) plays an integral role within the Integrated Team, gathering referral details from various sources including clients, their family members and other health and social care professionals, then sharing this information with the multi-disciplinary team to prioritise and allocate cases. The HSCC provides support to professional staff when dealing with urgent and critical community cases.

This is a part-time permanent post of 28.8 hours per week, to be worked in accordance with service requirements.

This role does not meet the eligibility criteria for a Skilled Worker visa or a Health and Care Worker visa. NHS Highland is unable to provide candidates with a Certificate of Sponsorship (CoS).
