About the role

Clifton Road care service supports young people aged who have been previously looked after in care. We provide accommodation and support in a residential setting with a range of services to meet individual needs in partnership with other agencies to improve life chances and support independent living.

As the Team Leader you will spend time supporting the staff team through supervision, team meetings and running a live rota, and you will also work directly with the young people following detailed care plans.

Responsibilities

  • Managing and leading a team of residential professionals and other staff as directed in the delivery of a 24 hour rota, including shifts, utilizing policies and procedures to enable staff to provide a responsive, high quality person centred service.
  • Building and maintaining positive relationships by providing emotional/social support to young people in a therapeutic way.
  • Leading case/care planning for individual children or young people: assessment of need, developing a care plan, identifying agreed outcomes, and carrying out regular reviews with children, young people, families, and other professionals.
  • Supervising, guiding or directing staff and volunteers where required and mentoring newly appointed staff.
  • Attending professional meetings including reviews, court work, etc., referring appropriate cases to specialists or higher management.

Qualifications

  • SVQ4 Leadership & Management and HNC social care qualification.
  • Ability to work on a rota basis and participate in on call rota.
  • SSSC registration.
  • Experience in planning, prioritising, and evaluating activities and own work effectively.
  • Demonstrated ability to make informed decisions and judgement, anticipating and identifying problems and finding solutions.
