About Us:

• Serving approximately 8,300 patients
• Team includes:
• 5 GP Partners
• 1 Advanced Nurse Practitioner
• 1 Practice Nurse
• 2 Chronic Disease Nurses
• Pharmacy team
• First Contact Physiotherapist
• Supported by an experienced administrative team
• Day to day operations overseen by the Practice Manager and Assistant Practice Manager

The Role:

We are seeking a motivated and experienced Advanced Nurse Practitioner (ANP) to join our dynamic team. As an ANP, you will play a vital role in managing a diverse caseload, working closely with GPs and other healthcare professionals to deliver high quality patient care. The role involves:

• Triaging
• Face to face clinics
• Home visits

Key Responsibilities:

• Assess, diagnose, and manage patients with acute conditions
• Perform clinical examinations and arrange investigations/referrals/admissions.
• Be responsible for management of own results and docman
• Ensure appropriate follow up arrangements put in place for patient
• Prescribe as per local guidelines

Systems:We currently use Vision and Docman for clinical and document management.
