Adaptive Home Health, LLC is seeking a Physical Therapist to own the caseload in our field-based home health program. You will deliver comprehensive PT visits, perform in-home assessments, and advance gait, balance, transfers, strengthening, and functional mobility with strong patient coaching and safety focus.
